Instructions:

  1. Completely fill out form and submit. After submission, you will be able to print a receipt.
  2. If possible please send in phone with charged battery. No need to send your battery charger.
  3. Do not send in the phone with your memory card or sim card.
  4. Ship us the phone along with a copy of your Service Request Receipt to:

  5. Once your phone is received it will be evaluated by a technician and you will be contacted with a diagnosis report and an estimate on the repair.
  6. If your phone is repairable and the repair estimate is approved, we will contact you after the repair is completed for payment processing.
  7. We will ship your repaired/unrepairable phone back once a payment is processed.
  8. We charge a $5.00 diagnostic fee for all phone evaluated. Most Shipping charge via United States Postal Services is around $6.00.
